What are the signs of a toxic workplace?

What are the signs of a toxic workplace?  For starters, a toxic workplace is not an illegal workplace.  Employers are free to treat their employees in disrespectful ways but to the point of the legal breaking point.  Employers can yell, scream and intimidate employees all day long without legal effect.  However, when the toxicity reaches the level of physical assault and discriminatory disparate treatment, then an employee can argue they are being subjected to discrimination via a hostile work environment.  A hostile work environment claim can apply to age, race, sex, sexual orientation, disability etc. and often stands alone as a separate claim.
