How do I prove a hostile work environment?

How do I prove a hostile work environment?  First, people talk about a hostile work environment and do not even know what it is.  Do not believe that every action by your employer will fit into the definition, because it takes much more to make your employer liable. In order to allege and prove a hostile work environment claim, an employee needs to show the following: (1) the work environment is objectively hostile and subjectively offensive; (2) you complained about the harassment to management; (3) the employer’s conduct was severe or pervasive; and (4) there exists a basis to impute liability to the employer.
